I am a seasoned advocate focused on person-centered healthcare transformation. I am known for my ability to collaborate, solve problems, and connect the dots. Having cared for multiple parents with dementia, I have a 360 degree view of healthcare, social services, and the challenges faced by patients and their families. My areas of focus include: health policy, health literacy, health content development, health IT usability, communication, and training. I serve as Patient Research Partner and Ambassador for the Patient Centered Outcomes Research Institute (PCORI), and Adviser for the National Alzheimer’s & Dementia Patient & Caregiver-Powered Research Network. I am a former Executive in Residence for the Healthcare Information Management and Systems Society (HIMSS) Foundation, Institute for e-Health Policy. My mission is to make healthcare and health research easier for patients and family caregivers.
AVAILABILITY: 21-30 hours/week
Connected Health Resources engages patients and families in health management, care transitions, medication management, ongoing treatment and recovery. The Community Health Gateway delivers plain-language and multilingual tools and resources to patients and family caregivers that improve engagement in chronic care and after acute-care facility discharge. The Gateway also promotes integration of healthcare, social services, and community supports – improving patient outcomes.
